WHAT IS HOME HEALTH?
Home health is a combination of medical services delivered in your home at the request of your physician when you have experienced a change in your medical condition. The job of the Home Health Professional is to teach the patient and/or caregiver how to care for the current medical problem.
AM I ELIGIBLE FOR HOME HEALTH?
Anyone is eligible for a free Home Health evaluation to determine their need for services. If appropriate, your physician would need to request Home Health Services on your behalf.
IN ORDER TO RECEIVE HOME HEALTH CARE YOU MUST BE HOMEBOUND.
This means you must experience a taxing effort to leave the home. Absences from the home should be for short periods and infrequent.
THE CARE YOU REQUIRE MUST BE SKILLED IN NATURE.
This means the care you receive can be delivered only by a skilled medical professional.
THE CARE YOU REQUIRE MUST BE INTERMITTENT.
WHAT TYPES OF CONDITIONS WOULD INDICATE A NEED FOR HOME HEALTH CARE?
• New surgical procedure.• New or change in medical diagnosis.• Difficulty walking, transferring, or performing regular daily activities.• New or changed medication.• Problems with talking or swallowing.• New medical equipment (oxygen, walker, meters).• New or changed caregiver.
