WHAT IS HOME HEALTH?
Home health is a combination of medical services delivered in your home at the request of your physician when you have experienced a change in your medical condition. The job of the Home Health Professional is to teach the patient and/or caregiver how to care for the current medical problem.
AM I ELIGIBLE FOR HOME HEALTH?
Anyone is eligible for a free Home Health evaluation to determine their need for services. If appropriate, your physician would need to request Home Health Services on your behalf.
IN ORDER TO RECEIVE HOME HEALTH CARE YOU MUST BE HOMEBOUND.
This means you must experience a taxing effort to leave the home. Absences from the home should be for short periods and infrequent.
